DO NOT FILL THIS OUT IF YOU HAVE A STUDENT CURRENTLY ENROLLED IN THE ACADEMY.  (For re-enrolling current students, please go to Registration/Re-Enrollment '24-25 under Quick  Links).
 
There is an application fee of $35 per student applying to St. Martin of Tours Preschool or Academy due upon completion of the form.  You may submit the payment to the Academy office. If choosing to pay by credit card or PayPal, a small convenience fee will be applied.  Preschool students  need to be three years old by Sept. 1st and fully potty trained.

The application process also includes the Receipt of Student Records 
-Copy of last 2 report cards
-Copy of previous standardized test scores (if applicable)
 -Birth Certificate
 -Immunization Record
 -Letter of readiness for kindergarten from preschool (if applicable)
-IEP/504 Plan (if applicable)
